Okay now, here comes the part where I explain, step-by-step, how to complete this project. You know just in case you want to make one. It is pretty simple however it does take up a lot of your time. So first off you take an image, note that if you want an easy one you should take a symmetrical photo so that you really only need to half of the picture then just flip it. You have to turn on the grid on your photo before you start the next step. So you get that into photoshop and then you take the line tool which is the third to last tool in your photoshop tool bar. You then take that tool and trace the outline of your head making sure that you end each line at a horizontal or vertical line in the grid. You take the lasso tool and select each one of those triangles and then fill them in using the filter>blur>average. That is how you make a poly portrait.
There are definitely other ways to do that though. You have to get an app to complete the easier way. However there are some cons to doing it the easy way. It ends up not being as realistic and you cannot see your face as well. Also, there are really no ways to customize your project.
